This website uses an essential session cookie to protect forms and authenticate staff. It does not use advertising trackers or third-party analytics.

Patient submissions

Submitted forms are stored for review by authorized clinic staff. Please call the office to request corrections, copies, or help with your records.

Staff sign-in is provided by the practice’s Sessionate service. Patient form contents are not sent to that service. Do not send medical details through ordinary email.

Notice of Privacy Practices

This website privacy information is separate from the practice’s Notice of Privacy Practices. Ask the front office for the current notice.
